Is A Circlage Procedure Possible When The Cervix Is Dilated 3 Cm?
Question: I required your opinion about one of the problem faced by my sister :
"Hello Doctor my name is xxxxxxxxx. I reside in XXXXXXX DC USA. Sorry to bother you at this time. As of today my wife is 20 weeks 5 days pregnant and she got bleeding this morning when went to our gynac she said that her cervix was dialated so they admitted her and wanted to try cerclage. But after they did ultrasound they came to know that she was 3cm dilated and there is no way they can do cerclage with no cervix.
But we asked if they can re-evaluate in the morning so currently she is on antibiotics and glucose and rested in a tilted position. I was wondering if we should try to do cerclage in this case or it would be better to let the baby go at this time and do DNC."

At 20 weeks, if the cervix is already 3 cm dilated then circlage is not possible. If after rest and medicines the pain does not subside and the cervix keeps on dilating then it is going to be an inevitable abortion. In such cases, D&C is usually not needed and the baby and placenta comes out spontaneously.
In case the pain subsides and cervix closes slightly then only circlage is possible. It is best right now to wait and watch for the pains to stop.
