Question: Hi Doctor,

My uncle, aged 63, heavy smoker in past, is presently suffering from LUNG CANCER METASTASIZED TO BRAIN.

He has undergone CORONARY ARTERY BYPASS SURGERY (CABG) three years before and his present EF is only 35%.

Our dilemma is that one part of family is suggesting to undergo CHEMOTHERAPY as suggested by our local cancer specialist and one part of family is suggesting to undergo palliative treatment or some herbal treatment inorder to reduce pain as they are against of going with CHEMO due to the underlying Heart Bypass.

His present symptom is Pain in Head which use to get relieved by pain killers.

As a very important second opinion, kindly advice the following on the basis of attached Medical reports.

(1) Based on the pros and cons of going thru CHEMO in his case, whether he should take the risk of undergoing CHEMOTHERAPY?

(2) How advanced can we say about the progression of Cancer in him?

(3) What is the likelyhood of another heart attack of heart disease due to CHEMO if we undergo the same.

(4) If upon your evaluation, the stage is advanced with bleak scope of recovery, what is your suggestion towards therapy. ie, CHEMO, or alternative medicines, or Palliative care etc.

(5) How long can an extension of life be anticipated with CHEMO than without undergoing CHEMO.

Looking ahead to have an answer from you.

Thanks and Best Regards.

Brief Answer:
Hello dear. Several options are there

Detailed Answer:
Hello dear. I have gone through the details. So to brief you.
Its a stage 4 disease and is very advanced.
Treatment option can be either oral targetted drug or chemo drugs.
Both treatment will be safe and we can choose chemo drug which are cardio safe.
He needs to undergo few mutation tests on biopsy. If mutation tests are positive, oral targetted drug can be given which are very easy to tolerate as compared to oral.
With treatment, life expectancy will be around 9 months.
Radiation to brain is also needed and safe.
Thanks and regards
