Question: I take Inderal 10mg for event induced anxiety - like before I need to perform, public speaking etc. For the past couple of years this has been working fine and no evident side effects except a mild headache.

In the past year and half - not related to Inderal I have had other health issues primarily a persistent dull left sided chest pain that comes and goes. This has been evaluated by cardiologist and they noticed ventricular premature beats and said nothing to be worried about and even prescribed Inderal for symptomatic relief. I have also had an upper GI endoscopy that came back normal. I am maintaining stable weight and do not have other issues except dull chest pains and extreme fatigue.

Now - i notice that on the day that I take Inderal I am fine. The next day when the drug wears off I notice my chest pain to be much more severe. The pain is a dull ache on the left side of chest.

Question - Can inderal cause this effect - why would it do this suddenly as it was working fine without any issues before? Is there anything else medically that can cause this kind of pain due to Inderal that I need to get checked out?

Does Inderal exacerbate anaemic condition and if yes can it cause chest pains when it wears off - Rebound effect? My last report MCV was 101.1 - macrocytosis due to low folate. I am taking supplements but no improvement as of now.

Dear sir, I have gone through your question and I understand your concerns but inderal rarely causes decreased cardiac output and worsening of chest pain on discontinuing it.. since you are not using it regularly , just on and off, you might be experiencing worsened chest pain and decreased cardiac output when it’s duration of action ends.. I would recommend that you get an echocardiogram if not previously done to asses your cardiac functions..
Inderal is not reported to have an effect on anemia, RBC count or on macrocytosis, however it can have a negative effect on platelet count on rare occasions..
Increased red blood count , could be due to dehydration, so make sure that you are adequately hydrated with at least eight cups of water daily..
Another causes of chest pain need to be excluded , including cholecystitis and costochondritis ( inflammation of rib cage joints) . This is why I would recommend that you get an ultrasound abdomen to assess your gall bladder.. costochondritis can be self diagnosed easily by pressing on the edges of your sternum. if tender , then costochondritis is a likely cause of chest pain.. I have done multiple echocardiograms and they have come back normal except for frequent ectopics. I have attached some reports as well.

I am curious to know why there would be decreased cardiac output when the Inderal wears off. Shouldnt there be decreased cardiac output while the drug is active and then cardiac output would be back to normal once the drug is out of the system?

Could the chest pain be pleuritic in nature - does inderal make respiratory conditions worse? Is there anything that you can recommend from a follow up perspective? I have tried the suggestions for costochondritis and it does not appear so - there is no pain while applying pressure near sternum. The pain does not respond to touch / palpation. It feels like coming from inside.

Please advise if you have other suggestions on what this could be. Suffering from chest pains for the last year and half.

Dear sir, hello again.. Inderal decreases cardiac output as a mechanism of action but rarely it causes the side effect of blunt cardiac output and further reduction as it wears off.. chest pain related to pleurisy is usually worsened by inhalation. Inderal can worsen pre-existing asthma or respiratory conditions but is unlikely to initiate them in a previously normal individual.
If you have undergone an echocardiogram and the results came back normal. Then you should get an ultrasound abdomen to exclude gallbladder conditions as a cause of a referred chest pain.
Also, I would recommend that you keep a record of your blood pressure while on and while off Inderal as it might cause paradoxical hypertension as it wears off. I hope you find this helpful

Thank you for the response.

I have undergone an ultrasound of the abdomen and this has come back as normal as well. I have BP reading taken multiple times and it is around 130/90 and 110/70(on inderal).

One other symptom that I forgot to mention was excessive burping. This is a fairly new symptom that I never had before. The excessive burping is not related to food patterns - happens even when on empty stomach from time to time.

Also should I be concerned about atypical angina. The pain I experience does not increase in intensity during exercise but I have some chest tightness AFTER exercise. I also feel like I am unable to take deep breath from time to time during activity. I am also very fatigued most of the time and am unable to exercise properly.

Please advise and thank you for your time.

Dear this, hello again.. If you experience chest tightness after exercise then I would recommend that you get a stress ECG test or a Holter analysis.. Since you clarified that your are experiencing excessive burping with the chest pain then this adds the possibility of colonitis , IBS , gastroenteritis or less likely pancreatitis.. I would recommend that you get a stool analysis , schedule for colonoscopy to exclude colonitis and IBS and get a blood test for pancreatic functions..
