Question: I am 5.8 high and i weigh 72 kg of age 24. I always been slim and skinny all my childhood, 3 years back I was diagnosed of Gynecomastia and since then i had developed fat over my abdomen area and chest but other parts of my body still remain skinny though. I don't drink alcohol nor smoke nor been on any drugs all my life. I look under grown and small compared to people of my age. Other symptoms which i had are day dreaming, not able to concentrate for long, getting tired easily, slight depression, not able to exercise, feeling week and these symptoms are effecting me in daily activities. Recently when I got an MRI of my abdomen for diagnosis of Cushing, it came out that I had 17 cm of fatty liver. The midnight cortisol results and some lab tests were border range, so my doctor suggested to go for low dexamethasone suppresion test. Results are attached.

what does this results mean, do i have cushing or not?
Do I need to take high dex test for further evaluation?
Please suggest...

Brief Answer:
About Cushing syndrome diagnosis,

Detailed Answer:
Hello and thank you for asking!
I have gone carefully through your concern and attached exams.
Low dexametasone suppresion test results negative for Cushing syndrome and cortisol in saliva is normal.
So there is no need for further investigation of Cushing.
I would suggest to check prolactine level if you did not check it and thyroid function.
They may be responsible for gynecomastia.
Please do not hesitate to ask if you have further questions.

But my thyroid also looks fine, report attached. I don't smoke or drink. I have always been under weight since my my childhood. I don't have proper facial hair unlike men from my family. From past two years I went from underweight to overweight and I only eat small quantities, normal meals. Since I was diagnosed with gynecomastia, my abdominal fat also increased and I had so much trouble with lack of concentration and I got the fatty liver from in the last quarter. If its not cushing what could these symptoms be?

Check testosterone:

Detailed Answer:
Hi again,
The symptoms are suggestive for low testosterone levels, a clinical examination and lab exams (testosterone and prolactine) should be done.
