Question: Hi Doctor,

My wife needs to go for laproscopy surgery for stones in gall bladder. How many holes will be made in the body for this surgery?

Hi.
Thanks for your query, albeit short and specific.

In the standard Laparoscopic cholecystectomy, 4 small incisions are made.

The first one of 10 mm is made near the umbilicus to gain entry into the abdomen and to pass a endoscope.
2 nd is done in the left upper abdomen for passing the operating instruments.
3rd is 5 mm in the upper abdomen and
The 4th 5 mm in the right part is made for holding instruments.

In another procedure (which is done at only a few places, a single incision is made in the umbilicus to pass and hold a larger instrument holder wherein all the scope and instrument can be passed. This obviously will be a larger but single incision. Called as SILS (http://www.covidien.com/surgical/products/single-incision-laparoscopic-surgery)

Hi Doctor,

Thanks for your reply.
The surgery of my wife is done today morning. Four incisions were made as suggested and the operation was successful. The stitches need to be removed after one week.

1. Could you pls suggest the precautions which need to be taken after this surgery?
2. When can she go back to work? The doctor has suggested one week rest.
3. What are the dietary precautions which needs to be taken now...let's say for initial one week and for initial one month after surgery?

Thanks for your feedback queries.
Nice ti know that the surgery was 4 port and successful. I also hope that the previous stye is all settled.

Yes, stitches or staples will be removed after 7 days.

1. Precautions to be taken after surgery:
Early movement/walking around to avoid deep vein thrombosis.
Maintain hydration properly by proper intravenous fluids and the oral fluids whenever your Surgeon allows.
Nothing special to worry about if the post-operative period is smooth.

2. With the advent of Laparoscopy, patient can go to work after 7 days. I is better to wait for 7 days as the healing of the 1 st stage is complete, the patient gets adjusted to post-operative life, mild pains and changes in daily routines.

3. Dietary precautions: now: water will be started followed by all liquids and soft bland diet (do not give any tasteless fluids or diet- a thing which is commonly done as one follows the advise of friends and relatives too like give this and do not give that).
The best policy which I adopt is to ask the patient what does she want to take and give those things (of course not the oily and spicy); add taste to whatever you are giving.
Later: only avoided are oily foods as there is no gall bladder to store the bile and it will be flowing continuously into the duodenum.
The best policy is to divide the daily food requirements in 4 parts and to be taken as breakfast, lunch, brunch in the evening and dinner.

She can have the normal food withing a few days.
Avoid outside food strictly- anything made at home please.

Thanks again for your reply. Really appreciate your detailed replies which are very helpful for us.
Few more specific queries. Though our surgeon has advised that she can take normal diet from 3rd day but it would be great if you can advise whether she can take following things after 4-5 days of surgery. She is having sore throat after the suregery.

1. Fruit juice
2. Curd/salad
3. Ghee/oil added in vegetable while cooking
4. Any specific fruit to be taken or avoided
5. Any particular vegetable or daal/rajma/chhole to be taken or avoided?
6. Is it ok to sit in office from 9 to 6 after 10 days of surgery when she will resume work. Any extra care that shall be taken?

Hi.
I hope your wife is doing fine.
Yes, let her start absolute normal, home-made diet from 3rd day. And as I said, ask the patient what she would like to have as due to the stress of anesthesia, surgery, the medicines, the atmosphere and all really make the patient to loose interest in eating.

Now the things to be taken after 4-5 days. The sore throat will pass off as she must already be under the medications that will act on throat too.

1. Fruit juice > whichever she asks for and suited her before the surgery- nothing new to be tried.
2. Curd/salad > Curds for taste and salads if suits her.
3. Ghee/oil added in vegetable while cooking > Avoid as mush as possible. To be used to cook only sufficient enough to give taste to the food. Ghee and oil should be avoided as I discuss in earlier discussion as the store-house of bile is not present now.
4. Any specific fruit to be taken or avoided > as discussed, anything which she used to take before surgery, nothing new to be tried.
5. Any particular vegetable or daal/rajma/chhole to be taken or avoided? > if these things suited her in past can be given.
6. Is it ok to sit in office from 9 to 6 after 10 days of surgery when she will resume work.> Certainly yes, if everything goes smoothly.
7. Any extra care that shall be taken? Try to avoid heavy lifting or any such activity that increases pressure on the abdomen for the first 3 weeks as by that time the healing is very much enough to hold on the tissues together.
No other extra care needs to be taken. She can withstand the procedure well.
