How Can Appendectomy With Pain At Incisions Be Treated?

Question: Hi,
My daughter had appendectomy 16 days ago. She has some pain at her incisions and surgical site that come and go still. However, she’s been taking only one Tylenol a day. She’s also having throbbing headaches at the top of her head that come and go. Sometime the pain oscillates between stomach ache and headache. Is this normal? If it is, how long do they last? How can this be treated?

She had a laparoscopic surgery under general anesthesia. No other symptoms are there except burning sensation in vagina and itchiness around vagina. A gram stain was performed on her two days ago and it showed intermediate for BV. Please see attached Attached is also the pathology report of her appendectomy and pic of appendix before removal. It looks like she had some inflammation going on for a while. Doctors performed diagnostic laparoscopy on her and found nothing else abnormal, which is when they removed the appendix.All of this started on Sep 5 when she had a diarrhea for four times during the day followed by burning sensation in vagina at night. From next day she started having lower right quadrant pain, which eventually turned into repeated attacks of burning and stabbing pain multiple times during the day in lower right quadrant . She was treated with antibiotics twice for uti ( one time she had efecalis 10-49k and the next time for klebsoella pneumonias 50-99k), however no one performed gram stain on her at that time as she’s a minor. They prescribed amoxicillin for 10 days followed by Ciprofloxacin for 7 days. However, She kept getting headaches and repeated lower right quadrant attacks and intermittent burning sensation in vagina in the past two months. The urine culture came back normal after antibiotics. Then again when she had burning sensation, it was treated with Nyastatin for a week. It went away and again now after surgery she got it again. So took her to emergency and made sure they did gram stain this time, which is when intermediate for bv was detected. However, the headaches were persistent and intermittent even after surgery. Docs chose not to treat for bv at this time. Her urine Ph was 6 and specific gravity was greater than 1.030. They said she may be dehydrated.

All her ultrasounds and mri of abdomen and head came back normal before surgery.
Doctors also said her incisions are healing fine. But she keeps getting sharp pains at belly button incision and surgical site on and off during the day.She does have some burping when eating post surgery.

Good to know MRI of brain and abdomen were normal.
How old is she?
Belly button incision can be painful as some small nerve can get entrapped. If there is no swelling and redness then nothing much to worry about.
BV is a troublesome problem. Many a times probiotics many help. Examination by a Gynecologist will be a better idea in spite the patient is a minor. Local hygiene is also important.
As for the pain in abdomen, this can be colitis or typhlitis or IBS. A thought should be reminded if further investigations are needed.
Very difficult to correlate between throbbing headache and pain in abdomen.
Dehydration can be the cause. Since spinal anesthesia was not given hence it is not a spinal headache.
Sometimes viral infection can cause such symptoms if all investigations are normal.
Stress and anxiety can be an instigating or enhancing factor in some patients.

Ok, the pain in the abdomen is only a sharp pain at the surgical site. Is this uncommon?

Thanks for the reports. Read all available now.
It is not common to have pain after surgery. As explained above other causes may be looked for.
Also watch for any foods or beverages that instigate or enhance the pain and stop those. This is very important. Intolerance to any foods or beverages can develop anytime.
Antibiotics change the flora of intestines and may need probiotics. But discuss with her Doctors before starting anything.
