How Can A Severe Bruise On The Right Leg Be Treated?

Question: i have a 25 -30cm bruise on my right leg from a bicycle accident
yesterday
I'm due to fly 9 hours long haul on Saturday
should i fly?
the bruise is still coming out as it were
with a big bolos centre thats not black
the outer edge is black

should i ice until its all black?

and still ok to fly in 2 days with it being so bulbous?

Thanks for your query.

To recapitulate: Female/54 - 25-30 cm bruise on right leg - bicycle accident yesterday - bruise still coming out - big bolos (?bullous) center - that is not black - outer edge is black - due for 9 hours flight on Saturday that is 2 days - wants to know whether to ice until it is all black and is it okay to fly in 2 days with it being so bulbous ...

Please give additional information for me to assist you better:
First of all, please post clear and focused photos as they help sometimes for on-the-spot diagnosis.

Which area of the leg is affected ?
Has this affected the movement of the leg/lower limb ?

Leg being the most dependent part of the body, far below the heart level the inflammation, edema and pain are the maximum. Hence the best position to avoid all these is to rest in bed with the elevation of the affected area.part above the heart level in laying down position.

Take an anti-inflammatory medication (eg. Ibuprofen 400mg twice daily after food or Tylenol 650mg thrice daily after food) for the time being.

Antibiotic can be suggested if there is a breach in continuity of the skin and application of liquid Povidone iodine to the whole lump to avoid secondary infection.

If there is so much edema this may not settle so early and 9 hours is a long flight and hence you can go by flight only if:

- there is a seat wherein you can lye down in such a way that the affected part can be elevated above heart level if the edema is so much.

- X-rays and clinical evaluation show no fracture and infection.

- Since the outer edge is black, it would be worthwhile to observe this keenly and to go by flight only if there is no further progression.

- If you take proper care and rest, this may settle to an extent within 2 days.
- Icing usually does not help much after 6 hours of an injury.

Ultrasound, aspiration, tight bandage

Hi.
Thanks for the photos, these are suggestive of a Hematoma.
Please go for Ultrasound and aspiration if hematoma followed by tight bandage; this will help to know whether this is superficial hematoma or whether there is some internal/muscular injury.
You may then go for the flight if there is no problem with movement of the affected part.

The center of the elevated area gets numb due to direct injury to the nerve fibers and/or stretched due to hematoma formation.
Muscles underneath can go into spasm as protective reflex or due to direct injury.
