Question: Hi, this is about my sister(28 F).. irregular periods since 2012(worsened with time that no period in 6 months also ), married in 2014, trying to conceive since married and found out to be PCOD in 2015. All reports attached, in concise, went through 5 cycles of IUI. For the first 2 cycles I had one good matured follicle on day 14 or 15 but my husband was low (first it was 8 mill/ml and next it was 3 post wash). For next 2 cycles (husband started on fertisure and spemen) with another doctor I had 2 follicles each cycle (3rd cycle it was 8 and 4this 60) but early at day 10. Then doctor suggested me to go through laparoscopy to find out if any minute reason that’s stopping me from pregnancy (laparoscopy on May 2018 on day 10). Findings were tubes open, no cysts, performed HSG meanwhile and no abnormalities. Even though doctors expect period to be on its own, it was delayed. So had to use Meprate to induce period.  5th IUI (June) 2 good follicles (at day 15) with 44 mill/ml count, currently in 2 week wait. Uterine lining is good in all cases. I am 5.4 inches (ht.) and 60kgs (wt.). So my questions are:
1.
Am in the right way of treatment? If so what's stopping me to get pregnant?
2.
What is the reason for my PCOD? ( my mom has diabetes)
3.
How could I correct my hormonal levels and get back my periods (un-induced)?
4.
Even though everything is fine with IUI why it is not working for me?
5.
what would be next best option other than IVF since we cannot afford it?

Brief Answer:
IUI has success rate of approximately 40%

Detailed Answer:

Hi,

1. The mode and way of treatment is alright, as in patients of infertility with PCOS, ovulation induction followed by IUI for 6 cycles is advised, before going for IVF.
However, IUI is successful in only 40% cases, and test cases result in failure. So, chances of failure are there.
2.PCOD is usually due to hormonal disturbance and insulin resistance.
It is common in patients with family history of diabetes.
3. Exercise, weight reduction, lifestyle modification like outdoor activities, lawn tennis, badminton, swimming, and avoiding fatty food along with metformin and myo-inositol
are the mainstay of hormonal management in PCOS.
4.IUI do not works for 50-60% of patients.
Cervical immunity, irregular motility of sperm, adverse conditions in endometrium are few of the many reasons for the failure of IUI.
5.If 5-6 cycles of IUI have failed, then IVF is the only option.
You can search for IVF centers which provide the services at low cost.

Hope you found the answer helpful.

Thank you for answering. yes I am working on lifestyle modification. lost some weight by following taking low carbs diet and exercising since quiet a long time . but I am more concerned about my period regularity and ovulation. and also my AMH shows decreased in these 2 years to 1.5 . so I'm bit worried also I started on taking cinnamon and fenugreek seeds in my diet as they could help my condition. but heard like they decrease the effectiveness of fertility drugs used. is it true?

is reduced AMH(from 2.8 to 1.5 this year) alarming?

Answered by Dr. Deepti Verma (8 hours later)
Brief Answer:
Decreasing trends

Detailed Answer:

Hi,

The AMH levels are reduced definitely, as the values tend to decrease with increasing years.

However, a repeat level should be done from a different lab to confirm the values.

Take care. Hope I have answered your question. Let me know if I can assist you further.
Regards,
Dr. Deepti Verma
