Question: Hi doctor,

i'm 27 and I have had phimosis my whole life without knowing it was a problem or unusual. I just found out this past March that it isn't normal for my foreskin to not retract so I went to my doctor and he prescribed triamcinolone cream. The first night I applied the cream on the tip of my penis and did some stretching and massaging for 20 minutes. Later that night (to my surprise) I was able to retract my foreskin for the first time. My foreskin retracted past my glans. During that week, I used the cream about a few more times and was able to retract even further past the shaft. I can retract my flaccid penis fully now but I still can't retract it fully while it is erect. I can only retract it past my glans when I have an erection and it does not retract any further past the glans. It just stays there sitting tight. i have not been using the cream much or doing stretching excersizes. I would say in the last 7-8 months I used the cream maybe 8 times and did stretching excersizes for 20 minutes for about 2 non-consecutive weeks. This is getting really annoying because I want to retract my foreskin freely and quickly. i really don't have time to be taking 40 minutes out of my day to be stretching my foreskin and applying that cream. I just want a quick and easy fix. I do not want any kind of surgery because I've had bad experiences with doctors this past year and I honestly don't trust them operating on me even with a simple circumsition procedure. There has to be a way for me to cure my phimosis without surgery right? I mean....it's just skin. It's not some super complicated disease I have that can't be cured through conservative management.

my phimosis has never caused any pain for me so i really don't think that should be a problem. Even if it is, I will look into other ways to make the foreskin looser. I do not even want to consider circumsition unless it is a life or death situation which I bet is very unlikely. Hopefully something can be created in the near future that can actually cure phimosis (by curing I mean still havng your foreskin but having it loose and normal).

One last question, would the steroid cream or the new tightness around the glans have caused any erection issues? I noticed that after i used the cream for the first time and retracted my foreskin for the first time I noticed fewer and fewer spontaneous/morning erections. Before I noticed I even had phimosis I would get morning erections every single morning from the time I hit puberty all the way up until I used the steroid cream. After retracting my foreskin for the first time I noticed I would wake up some mornings with no erections, other days with weak erections and then some days with normal erections. I would say now I get morning wood maybe once or twice a week. I told some of the doctors I've been seeing about this and they are saying that it is normal not to have morning erections as you get older but I don't necessarily think that's true because I am only 27 years old, not 87 years old. Plus I asked some random guys online who are around my age and older and they say they still get morning erections every single morning so why would mine be any different? My erections during sex and masturbation are a little bit different too so I am not sure if maybe when I retracted my foreskin for the first time and masturbated that maybe the tightness around the shaft was too tight and ruined my erectile arteries or muscles? I didn't feel any pain or had any swelling or anything but I am not sure It is just too big of a coincidence.

If you felt the prepuce to be tight when you first masturbated with it in retracted position then it might have been due to the tight constricting feeling around the base of glans and your mind being conscious of it and not allowing the penis to get fully erect out of fear. A lot of people feel that there erections are not as strong during sex as during masturbation. Most of the time this is due to performance anxiety and if one has a tight foreskin as well, that is an additional thing to be mindful of. So I guess being concious of a tight foreskin and worrying about that tight feeling around shaft may hinder a little bit with your erections.

According to my medical web portal, the following are the only diseases I am diagnosed with:
Psoriasis
Phimosis
Anxiety
ED
Varicocele
Overweight

I am not too overweight but I guess for my height I am a little heavy right now. I am 5 foot 8 inches and weigh a little over 180 pounds.

My doctor diagnosed me with anxiety because I told him I've seen a lot of different doctors for my phimosis and the ED that was caused after my phimosis diagnosis.

My varicocle was found during the 8 mnth period that I was experienceing inconsistent erection problems. I felt a lump one day and went to the hospital and one of the nurses felt my testicles and found a varicocele. It doesn't hurt or anything and I even did a semen analysis to see if it effected my fertility and everything came back normal. 113 million total sperm, 44% motility with 2.5 forward progression and 9% morphology. The only thing bad in the semen analysis was abnormal viscosity.

My doctors keep saying my ED is psychological but they didn't do any tests other than checking my testosterone whcih was 425. In the notes my doctor wrote "patient says there is a decrease in AM erections. Most likely psychological. Possibly due to stress & age. Will monitor".

It kind of makes sense that this is psychological and due to my phimosis because when I first found out about my phimosis back in March I was extremly sad and thought about it for hours upon hours each day. But is it also possible that when I pulled my foreskin back for the first time and it was still really tight around my glans when I had an erection and ejaculated that the tightness around my penis during the ejaculation damaged my arteries or the veins inside the artery?

i did a full blood test and everything is normal as far as hormones go

I did try one the 2nd day I had this problem (I think it was cialsis) and I got a hard and long lasting erection. Although, I did take a stendra last month and got a semi-hard/softish erection. It won't really boost my confidence since I want to know that I can get a nice hard and long lasting erection without a drug. Getting a hard erection with the help of medicine is hardly what I would call reassuring me. The main problem with me is my penis feels numb in a flaccid state. Not numb where if I touch it I can't feel anything but numb in the way that I can't really move it too much anymore when it is flaccid and it feels lifeless and dead.

An a similar note, hat happens to the scrotum during an erection? My scrotum always becomes tight and doesn't dangle anymore but I noticed other mens scrotums during an erection still dangle like they do in a flaccid state. Also, yesterday after I ejaculated i noticed my testicles feel a lot smaller which is something I never experienced before. Thye are normal size again now but it was just scary. They felt like they disappeard. I felt it again today when I had an erection during sex. The testicles felt small and almost like they disapperaed. Just curious if this is something to worry about.

There has to be a way for me to cure my phimosis without surgery right? I mean....it's just skin. It's not some super complicated disease I have that can't be cured through conservative management.
