Question: Hi,

I’ve been having headaches consistently for two weeks.

1. The headaches fluctuates between temple pain on both sides (tension headache) and from ear to ear behind the head.

2. These two different headaches never occur at the same time. But one comes and it is followed by the other.

3. Other symptoms have included mild nausea (2 out of the 14 days), neck pain (6 of 14 days). Dizziness (7 of 14 days). Ear and cheek pain (8 of 12 days).

No fever. No other sick symptoms

I went to the Primary Care doc who suggested an ENT. The ENT said no sign of ear infection. He suspects either a sinus headache that needs the inflammation to go down or TMJ (temporomandibular join)

Treatment thus far. Nasal spray and anti inflammatory. Both work to relieve pressure and lower the headache pain from a 5 to a 2 or 3.

I want a second opinion and recommended next steps. Neurologist?

Brief Answer:
In my opinion next step should be imaging studies.

Detailed Answer:
Hello again.

I have read your new question.

Since the ENT Doctor suspected sinusitis, in my opinion, the next step to follow in your case should be imaging studies.

In my opinion, thin cut CT scan of your head and sinuses, including temporomandibular joints is necessary for a correct diagnosis.

The CT scan can evaluate and exclude any neurological conditions too.

Hope I have answered your question.

Feel free to ask if you have further questions.
