I attaching my father report who got pacemaker operation two days back.he got angiography done. I'm attaching the report plz read and suggest what should be done.
Past medication epilive 500
Blood pressure medicine tenoric
Symptoms brethlesness
Passing out
Low heart rate 40bpm not always pacemaker implanted

This is Dr Sameer, cardiologist from XXXXXXX
I can see that your father had giddiness and the ECG showed 2:1AV block and thus pacemaker implantation was done.
Coronary Angiography was also done which showed Triple vessel disease.
So, I wanted to ask whether he has giddiness now or is it ok now. Also please tell me whether he earlier had any chest heaviness on exertion.
He will have to be kept on blood thinner (Ecosprin) for now and will need intervention (PTCA or CABG) in case he is symptomatic as the blockages are significant. You have both the options as the blockages are localised and stenting can also be done in it.

It's too early to talk to talk about giddiness as of now he is still in the hospital. He will be discharged tomorrow. Dr asked me to come and meet him tomorrow for detailed further course of action. He didn't said chest heaviness but he did said about brethlesness and unable to walk and extreme tiredness before. He was sleeping very much before operation. Though I must admit he is bit lazy person and doesn't want physical work. I m worried very much plz tell is stent would be sufficient or bypass surgery would be needed. Plz explain me in detail about blockage. What is the severity of blockage. Is meditation alone would not be sufficient

Welcome back.
Sorry for the late reply.
These are 2 different types of diseases, let's discuss both one by one.
1st is the heart block for which your father has been put a pacemaker for. It is a disease of the normal pacemaker of the heart which produces electricity for the heart to pump (function). The symptoms are giddiness, dizziness or loss of consciousness.
As you are telling that your father was having extreme weakness and sleepiness, that is also a sign of low heart rate resulting from pacemaker dysfunction which causes low blood supply to the brain and hence the symptoms.

Now coming to the blockages, these are blockages in arteries which supplies blood to the heart. Any blockage more than 70% is significant and causes symptoms such as chest heaviness or shortness of breath on exertion.
In your father's case the blockages are significant and as you are saying he had breathlessness. So they should be opened up to relieve those symptoms and prevent heart attack (which occurs when a artery is 100% blocked).
The options he has are Angioplasty or Bypass surgery.
Generally if all the 3 arteries have blockages, treatment of choice is bypass. But some people do not want surgery and for them stenting (Angioplasty) can be done.
As the blockages he has are localised and not in the whole of the artery (diffused), angioplasty can be done and is a good option.
It is not a emergency as of now(as I can see from the report) and can be planned.
Now about medical management. Yes he can be put on medication and that can be tried initially and then we watch him for any symptoms. If the symptoms persist or increase, than we can go for angioplasty.
