Question: hi. i get alot of heart palpitations. i feel it in my throat and a thump im my chest. my doctor said its from anxiety or panic attacks that i do have often.

i ahve done a ecg. cardiac enzyme test. ultrasound of the heart. echo cardigram and a ecg stress test on a tredmil. please help

     Irregular heart beats need to be DOCUMENTED by ElectroCardioGram (EKG). Standard ECG records the electrical activity of the heart, for a few seconds. It can detect the arrhythmia occurring during that time only. If the episode occurs randomly or precipitated by a specific activity like exercise, one needs Holter or event monitor. Holter records for days; Event monitor for weeks / Holter records continuously / Event monitor only at certain times .
     It is reassuring that the investigations have practically ruled out structural heart disease.
The feeling depends on individual sensitivity – some do not feel them at all / some may feel every beat.
     Specific treatment may not always be necessary. Many cases of palpitation are benign , especially when associated with normal ECHO and TMT like yours and may not require treatment. If you also have symptoms like blurred vision, dizziness, loss of consciousness, shortness of breath, chest pain along with this, you need urgent work up
     The first line medicine is generally beta blockers, like Metoprolol – slow release and small dose. It will benefit both anxiety and missed beats. Your doctor will give proper prescription and follow up
     If they are not effective and if the problem is severe, You may need Electro Physiological Studies (EPS) / several other classes of drugs are also available – acting by different mechanisms - the specialist will decide tailored to your needs.
