Dear Doctor, So Here Is My Story: 1. First Day

Question: Dear doctor,

So here is my story:

1. First day of period - XXXXXXX 12th
2. In July found out I was pregnant
3. Found out it was ectopic.
4. 07/28 - Had surgery to remove pregnancy from follopian tube(laparoscopic) -Tube was not removed;
5. HCG was not going down so on 08/08 had one dose of a methotrexate shot;
6. HCG levels still not going down, internal bleeding was present. On 08/10 had a 2nd surgery to remove the tube (laparoscopically).

Now its been over 2 weeks since the shot and the 2nd surgery and on 08/25 me and husband had intercourse (went to a party, drank a bit). He used the pulled out method. I am now going beside myself if there is any possibility I could be pregnant from this one encounter. My questions are:

1. How likely is it that my body still had this methotrexate drug in the system at the day of intercourse and would it have any negative effects on the baby if it was 2 weeks past the shot?

2. The following day post intercourse in the afternoon I took an ovulation test (cleablue digital) and have been taking it for the past two days twice a day. All times, the test displays (low fertility) empty circle. Could I have ovulated on the 25th and somehow the following day the test displayed an empty circle already? Or is that not possible?

3. I would like to XXXXXXX my hair, I would assume that Thats not a problem?

*Please note I had my blood hcg levels measured on August 20th, and my level was at 58.

Firstly, with the serum beta-hCG levels at 58 and less than three weeks from the shot of methotrexate, ovulation is very unlikely. Though some people can experience return of menstruation 2 weeks after the drug, it is very rare and the average time of onset of menstruation is 6-7 weeks after the drug intake.

This is the most likely reason for the negative ovulation test. Also, the ovulation tests only indicate the LH surge and not the actual ovulation, which can happen anytime within 36 hours after the surge.

Please make sure you use protection until six months to avoid unwanted trouble. Ideally, you should wait for one ovulation cycle before trying to get pregnant.

Ovulation is likely to return as the serum beta-hCG levels come down to near zero.

I would suggest you wait for things to settle before you XXXXXXX your hair, though in the non-pregnant state it is of no harm.

Thank you doctor. I guess I am still missing an answer to my question 1. In addition, what I meant by the ovulation testing is whether or not an LH surge could have been there say on August 23rd or August 24th (day of intercourse) and when I tested on August 25th, it was already gone based on the result I got?

Further, I took a pregnancy test(early response digital) on August 25th in the late afternoon and that test said ”No ” so I take it that my hcg levels have gone down where the pregnancy store bought tests don't detect.

I do not think any LH surge would have occurred on the 23rd or 24th of August as it is too early after the methotrexate shot.

I would prefer blood test to detect the hCG levels as it is more sensitive. As you said, urine tests cannot detect low levels and this is why blood test is important. The home pregnancy tests bought over the counter have different sensitivity and show result at different hCG levels.

Early response digital test is supposed to be a good method of early testing, which can detect lower levels of the hormone also, when performed properly. You can repeat the test after a week to see if there is any change.

Dear doctor,

If LH surge would not occur any day before the intercourse on August 25th, then I am relieved because that's the only one time I had intercourse since all the procedures and therefore I am relieved to know pregnancy did not occur. This stress is too much for me right now so I will make sure any future intercourse until 6 months have passed is protected.
