Question: Dear Doctor,
This is regarding my son Hamshad
Age 6.8 years
Weight 17.4 kg

Actually yesterday after he came from school my son felt a bit lazy and tired.
At night around 10pm he was a little warm so we gave him a dose of Crocin syrup 5 ml and he went to sleep.
At night around 1.30 am he started to feel cold and was shivering as he had fever around 102.
So I gave him Crocin Ds and he felt good and slept well.
In morning again around 11 am he was a bit feverish so I gave him a dose of Crocin.
His appetite is a bit low now.

Later I took him to doctor and he advised the following tests.
1.CBC
2.Malaria test and Also Spot Test
3.Dengue
4. C Reactive Protein
5. Urine routine

Now I have attached all temporary reports and in reports C Reactive Protein is showing high so what does it mean and I hope other reports are normal as they look so.
Final report will come tomorrow.

So what could be the reason for fever and what should I do now.

I have gone through the query, have seen the reports and do understand your concern.
Viral infection is the most common cause of fever in this age. In viral infection fever is usually of high grade for initial 3-4 days but in between child remains well. Fever may be the only manifestation during initial days and child may develop cough or cold thereafter.
I have gone through all the reports. Except for C reactive protein and total leucocyte count all findings are normal.
Low total leucocyte counts (3200) is commonly seen in viral infection. Less commonly it can be seen in bacterial infection but in bacterial infection neutrophil counts increases remarkedly; which is normal here.
The only thing is borderline high C reactive protein. High C reactive protein suggests bacterial infection.
In summary, after going through the clinical picture and reports to me it appears to be viral infection. Raised CRP(which is not very specific for bacterial infection) is dubious. I would like to get serum procalcitonin level done which is more specific for bacterial infection.
Monitor temperature and continuing giving paracetamol for fever.
Observe for clinical symptoms and if fever persists even after giving paracetamol or if condition worsened please consult your pediatrician.
Please let me know the report of procalcitonin.

Dear Doctor,
The fair reports came today and in one report it shows positive for dengue and rest reports of dengue it shows negative.
Pls guide what does this positive mean??? as we are out of town and worried.
I have not give him any Crocin or ibugesic plus since morning as he is not having fever.

As per your advice I have done retest
These are his latest reports.

It shows
Dengue NS 1 antigen Negative

Dengue Antibody IGM positive 62.8
And
Dengue Intibody IGG Negative 3.65

So the IGM is high and is it a thing to worry. Pls advise.

Platelets 217

WBC count is 2540 low

So how many days this fever will stay and what to do for this low WBC count.
How to manage Dengue fever.
