Question: Dear Doctor,
This is regarding my son Hamshad
Age 6.8 years
Weight 18 kg
G6pd normal
Allergic levels high Serum Ige 3290 in September

Actually on 23 October after he came from school my son felt a bit lazy and tired.
At night around 10pm he was a little warm so we gave him a dose of Crocin syrup 5 ml and he went to sleep.
At night around 1.30 am he started to feel cold and was shivering as he had fever around 102.
So I gave him Crocin Ds and he felt good and slept well.
Next day in morning again around 11 am he was a bit feverish so I gave him a dose of Crocin.
His appetite is a bit low since birth and now it's the same as low.

Later I took him to doctor and he advised the following tests.
Test conducted on 24 October
1.CBC
2.Malaria test and Also Spot Test
3.Dengue
4. C Reactive Protein
5. Urine routine

Now I have attached all reports and in reports C Reactive Protein is showing high, Low WBC count and Dengue is positive.

Later he was ok and doctor told to keep him hydrated and we tried to do the same.
From 25,26,27Oct there was no fever and he was doing ok.

But sudden today 29Oct he developed fever in morning and we did repeat CBC and dengue tests.

My worry is The WBC count fell low from last report and platelets reduced a bit.
Also his Wbc was 6.67 in September and now what could be the reason for such fall.

Please guide what to do for the same. Feeling very tensed.
His food intake is low and he is week.

I have attached reports of 24 Oct,29Oct and previous Random CBC report of 12 Sep 2019 for comparison of WBC count

Brief Answer:
The WBC count and low platelets will recover as infection is controlled

Detailed Answer:
Hello,
Thanks for posting your query on HealthcareMagic.
As mentioned by you that your son is suffering from dengue fever.
Dengue fever is caused by bite by a mosquito.
Dengue fever is self limiting infection which requires symptomatic treatment only.
Since dengue is a viral infection, there is transitory bone marrow depression, leading to low white blood cells and low platelets,
The total leucocyte count ad platelet count will recover as the infection is controlled.
In the meanwhile, your son should be given balanced diet and adequate hydration.
If you have more queries, I will be glad to answer.

Dear Doctor,
Thanks for reply.
These are his latest reports.

It shows
Dengue NS 1 antigen Negative

Dengue Antibody IGM positive 62.8
And
Dengue Intibody IGG Negative 3.65

So the IGM is high and is it a thing to worry. Pls advise.

Platelets 217

WBC count is 2540 low

So how many days this fever will stay.

Brief Answer:
The blood reports show that infection has cleared.

Detailed Answer:
Hello and welcome again,
A negative NS 1 and dengue Ig M positive is indicative that the infection has cleared and now protective antibodies have appeared in blood.
The platelets have come back to normal level.
The white blood cells are slightly low, however, they will also come back to normal range within few weeks.
Since, the infection has cleared, fever will also fade away soon.
Nutrition and hydration should be maintained.
Rest should be taken since dengue causes lot of weakness.

Thanks and take care
Dr Shailja Puri
