I am Klerida Shehu gastroenterologist.

Yes, liver metastasis from colon cancer can cause loss of appetite, nausea and vomiting.

Other symptoms of liver metastasis are jaundice, abdominal pain especially in upper left part, confusion, fever and weight loss etc.

In cases of colon cancer, If vomiting Is profuse and large amounts than bowel obstruction should rule out firstly.

My dad has liver mets . After 3 chemo rounds in hospital since 2 weeks as has fever n chills .

Got put on iv antibiotic . Ct showed severe bowel inflammation .please see report .

He had fever plus nausea/ vomit .

Since on hosp bilirubin increasing as

Alt and alp are normal

Cea was dropping on chemo and shows the cancer fairly stable . Largest liver mets is 8 cm n 3 liver mets in total .

If chemo has not worsened disease much can liver b affected n bilirubin increase due to this size met ?

Thank you providing more medical data.
I read carefully your query, the report attached and I can say as follow:
1.     High bilirubin level may be due to -side effect of chemotherapy
-bile flow obstruction from largest metastases/liver failure

2.     It is a good sign that Cea level was dropping; it means that cancer is stable.
3.     With regards to vomiting/nausea it has been from bowel infection.
I suggest to :
1.     Repeat ALT, AST , ALP and bilirubin after a week.
2.     Perform abdominal ultrasound/MRI

The ultrasound was clear and also no bile duct blockage .

I am glad to know that there is no bile duct blockage and AST, alt are normal.

You are reporting too that your father has high fever, bacterial bowel infection.
In rare cases bacterial infection especially septic shock can increase bilirubin level due to bacterial toxins and inflammation substances.

Unfortunately still exist the possibility of liver damage from metastases and decrease the ability of liver to conjugate bilirubin.

So it is important to follow bilirubin level after antibiotic treatment. If at the end of antibiotic treatment bilirubin started to decrease than the high level of bilirubin is due to bacterial toxins.

Wish him fast recovery.

Ok we will try to test agsin now that he's off antibiotic .

What do you think re this pattern is it normal to shoot like this is was metastses or could it be antibiotic ?

Hi back,

1. With regards to high level of bilirubin, yes it can damage the brain. His bilirubin level is not too high, so don’t worry for the moment by bilirubin damage.

2. In rare cases liver damage from metastases can start only with high bilirubin level. Metastatic cell can't conjugate bilirubin because they don't have the ability of normal liver cells.

3. I need to know what type of antibiotic he has used. Not all antibiotics has side effects like increase bilirubin level.
