Can Metpure Be Taken When BP Reading Is 145/95?

Question: i am taking natrilix sr 1.5 once a day
metpure xl 50 once
for last 1 month. but bp still remaining 145 95.
now doc advises me to take S Numlo 5mg instead of metpure.
is this ok?

Hi, I am Dr Anshul Varshney , I have read your query in detail , I understand your concern.
Your BP is not very high, but still on medications it should not be more then 130/80 mmHg.
You can stop Metpure XL as it doesn't have a good BP control activity.
Carry on with Natrilix SR 1.5 once a day and start S-Numlo 5 mg once a day. Hopefully, your BP will get under control.
I would recommend you life style modification. If your BP further increases or doesn't get under control with lifestyle modifications, you would require low dose diuretics like Hydrochlorthiazide.

I would advice you following for life style modifications:
1. Reduce your weight. Start exercising regularly particularly brisk walk.
2. Avoid Fatty food in diet.
3. To consume potato, rice, oil, ghee, sweets, carbonated beverages in moderated amount only.
4. Avoid alcohol and smoking in case you do.
5. Take low salt diet and avoid pickle, namkeen, papad in diet

Thank you doctor. I would tell a little bit more to get a follow up reply. I have been moderately hypertensive for the last 2 to 3 years now. Being a pilot, I have to undergo routine medicals. Have been on nebi 3.5 on advice just before my medicals for the last 3 years. But now that doesn't seem to help. In August 2014, my bp came as 180 80 during examination. I went for an ambulatory, and there the results were satisfactory, although marginally. To avoid recurrence, and to get over this properly, I have now, for last 2 months started taking regular medicines, as told in the last query. I have reduced salt intake to bare minimum. Alcohol Has been reduced to occasional now. And I had left smoking two years back.
Regular medicines has not reduced my bp significantly, but the side effect has been that I think I am getting erectile dysfunction. Sex life has come to zero almost, from being very active till a few months back.
I am also taking Ayurvedic dietary supplement, two capsules per day for heart health.
In August, I have to undergo medicals where I will have to do TMT, lipid, sugar tests as well. Today's bp is 145 95.Above 140/90, I lose my flying fitness, besides being a long term threat to life.
Now, my questions are,
Will my TMT be affected by these medicines?Or I need to stop medicines before TMT.
Is there anything I can do for ED problem? Will Viagra help, or will it interfere/have side effect? Does any of these medicines contain nitrates?
Should I continue the present dose of natrilix and s numlo 5, and if yes for how much time before getting to stronger medicines?

Get Doppler penile vessels.

Hi,
Well optimal dosage decision for BP control takes time. As in your case you have achieved the optimal BP. You just need a slight modification of dosage as discussed.
None of these two medicines contains any nitrates.
As far as ED issue is concerned, you should first of all get a Doppler of penile vessels done to rule out any evidence of atherosclerosis in the penile vessels.
As far as the dosage and duration of therapy is concerned, you should start with these medications and get your BP checked again after one week and write back to me with your BP readings. Make sure that you have taken the medicine on that day.
