Yes, Hydroxyurea can cause leg and feet swelling because it affects the kidneys and causes chronic drug induced kidney failure which leads to fluid accumulation which is seen as swelling of the legs and feet.

Alone it is not known to cause any nerve damage or neuropathy but when administered along with antiretroviral medicines, it is reported to cause peripheral neuropathy.
