Can HIV Be Contracted Through Protected Intercourse After PRP Injection?

Question: Hi Doctor,

I had sex with a prostitute at XXXX 5.16.2015 14 days ago . Before I had sex, 15 hours prior I had undergo a procedure called PRP to my penis. The Dr Injected my penis with PRP. I did wear a condom when having sex but it was not pulled all the way to the base. Is there a chance that the needle injections were not healed after 15 hours and vaginal fluids can enter my body causing a me to get HIV? 2 days after exposure I got a rash with itchy bumps all over and got prescribed scabies cream and oral medicine (Ivermectin) for scabies. (Oral meds taken on 5/23/2015. Around day 9 of exposure I started getting sever headaches, stomach aches, diarrhea, a red pimple like painful on my left armpit, penile whit spot burns like yeast infection (last night), And I feel like my lympth nodes under my neck hurt but they are not swollen nor hurt when I push on them. I am concerned because the condom did not cover all the way to the base were the doctor injected me with prp 15 hours earlier. I do have a PCR RNA test scheduled for saturday 15 days after exposure. If you have any insite be appreciated. Thanks

I am also taking the 4th generation HIV test with the PCR test . Are these test reliable at 15 days? And should I follow up with another type of test ?

The needles used in PRP procedure are tiny. They do not cause any open wounds.They will heal immediately within minutes .So the chances of transmission of HIV is almost nil.Moreover condom is used.I do not see possibility of HIV transmission with given description. The person may be negative too.

The symptoms are non specific to HIV.Scabies can occur in any individual.The other symptoms are also not specific.Usually symptoms take at least 2-4 weeks to appear if infected with HIV.

HIV RNA PCR is a very sensitive test 10-12 days post exposure.You can go ahead with the test at this stage to relieve your anxiety.

Hi Dr Bharatesh,

Thank You for taking the time to answer my question.
Just to clarify the PRP needle injections that were done 12 to 15 hours earlier even if they were not covered by the condom have a low chance of contracting HIV? Would 12 to 15 hours give the needle injection spot significant time to heal. When I got the injections I did notice some blood on my penis were the injection sites were.
The area that concerned me the most was near the base because condom did not cover it (It was exposed) and I was all the way inside the woman. Also is the HIV RNA PCR reliable test? Also will 4th generation be accurate after 15 day exposure. Would you suggest another test at 3 months. Thanks again for taking time answering my questions. XXXXXXX

Yes.The tiny prick injuries formed due to prick will heal by fibrosis usually within minutes.They will not allow the HIV virus to be transmitted even if exposed.So the chances of transmission of HIV is almost nil.
Usually HIV RNA PCR detects HIV1 material.It is >99% sensitive. HIV 2 is very rare.So it is almost conclusive after 15 days.
4 generation tests are accurate after 3 months.
It will be better go for antibodies test after 3 months to be conclusive.
I do not see possibility of HIV transmission with given description.
