Posted on Wed, 8 Apr 2015
Question: Doc,
4 months ago had oral sex unprotected with csw from Thailand.. After one month had severe HIV symptoms thought test show negative.
I never got sick and visited hospital for last 3 years and ate healthy, no sugar and daily green tea , etc.
Symptoms:
2nd week - 1.) harshness of throat, went away next day
4th week - 2.) congestion in throat , 3. )White patches - till now for 3 months
8th week - 4.) Severe brain fog and no concentration till now for 2 months. Quit the software job and was best performer before.. 5.)full body tired and till now for 2 months. 6.) Red blisters on hand and chest - went away after 3 days.
10th week - 7.) joint pain and till now for 1.5 months. Getting sound while walking. Never had before .. 8.) Cold and till now for 1.5 months. 9) problem in breathing sometimes due to nose block.

10.) 4 kg reduced between 4 and 7 weeks and regained later.
11.) Got mouth ulcer thrice in 4 months.

It is more than 16 weeks and still have all symptoms excepts mouth ulcer and red blisters..

HIV tests:

4th week - Elisa - negative
7th week - cmia 4th gen - negative
7.5 week - rapid test - negative
8.5 week - HIV duo test - negative
10 week - rapid test - negative
11 week - viral load pcr RNA test, non detected
12 week rapid test - negative
16 week - rapid - negative
16.2 week - Elisa 3rd antibody - non reactive, hcv antibody - negative

Cd4 test:

7.5 week - lymphocytes - 17% , absolute cd4: 550
8.5 week - lymphocytes - 20% , absolute cd4 : 798
11 week - viral load pcr RNA test, non detected, absolute cd4 : 330.
15 week - absolute cd4 - 490
16 week - lymphocytes - 28%

Also the complete Wbc was more than 13000 during 7 weeks and now it is dropped to 9400.

Can you please advise me whether this is due to HIV some rare subtype and not deducted from the tests . I never got sick like this before and looks like something wrong with me..

Also the cd4 fluctuation is normal for HIV negative ppl?

Brief Answer:
You are HIV free beyond any doubt.

Detailed Answer:
Hi XXXX,
Welcome to HCM.
Thanks for posting your query.

First of all believe that you are HIV free. Repeated negative antibody tests even after 12 weeks past your high risk contact and undetected viral load confirm that you are HIV free. Don't imagine odd possibilities like rare sub type of HIV undetected by these tests. There is nothing like that.
There are umpteen number of causes for your symptoms other than HIV. You had already regained the lost weight.Breathing problem due to nasal block, sore throat, body pain, mouth ulcers can occur to anybody without HIV.
Fluctuations in CD4 count is also possible from lab to lab and also due to various other illness.
It is the challenge to your physician to find the exact cause for your problems through examination and investigations and treat you.
Get well soon.

Welcome back.
Sorry if you hurt on using your name. I can not delete the same from the previous answer from this end. I promise that I will not use here afterwards. But it appeared already in every question of yours.
You can go for the following tests:
1. Throat swab and culture and sensitivity test
2 ESR
3 ASO titre
4 RA factor.
5 Urine culture and sensitivity test
6. Tests for Dengue, Influenza and Chikunkunya and others as per your treating physician's suggestion (Widal, malaria etc)

Brief Answer:
ASO titre abnormality has to addressed. Get well soon

Detailed Answer:
Hi,
Welcome once again.
Your ASO titre is not normal. It reflects a Strepto-coccal infection. That could be a reason for your joint pain also. A course of Penicillin injection after test dose could relieve your symptoms. Consult your physician and get well soon.
