Have Osteoarthritis. Taking Thyronorm, Fefol. Suggested Knee Transplant. Advice?

Dear Sir,
My Mother name is Arati Saha. She is 64 yrs. She is suffering from OSTEOARTHRITIS. Her both knees are affected. But Right knee has maximum deterioration. Doctor has asked for Right knee transplant.
Current Test Report: Her BP is 145/80. Hemoglobin 9.50. PCV 32.10. RBC Count 3.39. MCHC 29.70. RDW 18.50. Other Blood Count Parameters are normal. No Blood Sugar. Serum Creatinine & Serum Uric Acid is also Normal. Serum Calcium 8.40, Serum Phosphorus 4.96. Triglycerides 178.00, VLDL Cholesterol 35.60. Other Serum Lipid Screen parameters are Normal.
Presently Thyroid Profile is Normal, but earlier its parameters were high so she takes THYRONORM-50 once a day. Earlier it was Thyronorm-100, later Thyronorm-75. For Low Hemoglobin recently she is taking FEFOL-Z twice a day after food.
Her Right Knee is in acute pain & quite difficult to walk. Presently except THYRONORM-50 & FEFOL-Z she is not taking any other medicine.
Please advice.

I am so sorry to hear about what your mother is going through.

If all medical treatment has failed to provide her with the much needed pain relief and functionality off her knee, then going in for surgical management is a good choice. From the information you have provided, I will recommend this.
As for her blood tests and the thyroid levels, i think her drugs will offer amelioration of parameters.

Hello,
Welcome to Healthcaremagic.
Before surgery, First thing is to correct anaemia.
For that she is taking Fefol-Z

With osteoarthritis, She also affected by Hypothyroidism.
Now she is getting Thyronorm.

Knee replacement is major surgery.
It has complications like Deep vein thrombosis and infections.
Both are difficult to treat at this age.
In addition, she have anemia and hypothyroidism.

Surgery is a stressful condition. At that times, the demand of thyroxine is more. If not supplemented in proper dose, it may cause fatal complication like Coma.

My advice is better follow Pain management with intra articular steroid injection etc..

Rethink and get second opinion from another orthopedician.

Generally, Total knee replacement surgery is the last option in treating osteoarthritis of knees. This is usually adviced for patients who have not responded for conservative line of management with over 6 months of treatment.

The conservative mode of management for OA knees is as follows -

* PHYSICAL THERAPY : The mainstay of treatment in the early case is physical therapy, which should be directed at maintaining joint mobility and improving muscle strength. Other measures, such as massage and the application of warmth, may reduce pain.

* LOAD REDUCTION : Protecting the joint from excessive load may slow down the rate of cartilage loss. It is also effective in relieving pain. weight reduction for obese patients, wearing shock-absorbing shoes, avoiding activities like climbing stairs and using a walking stick are worthwhile.

* ANALGESIC MEDICATIONS : Simple over the counter medications like acetaminophen is sufficient in most cases. If this fails to control pain, a non-steroidal anti-inflammatory drugs may be better.

* VISCO SUPPLEMENTATION : Hyaluronic acid injections given into the joint in prescribed dosages will give good short term relief in selected cases and helps in postponing total knee replacement surgery atleast for a year or two.

If all the above measures fail to provide pain relief, then a Total knee replacement may be needed.

Thanks for writing to us.
To have relief in osteoarthritis knee,you mother should do following-
-Start doing exercises for quadriceps muscles.she may learn this from physiotherapist.
-Start doing warm fomentation with rubber bag over knee for at least 3 times a day.
-Take a good analgesic like advil or aleeve.
-There are some dietary substances like glucosamine ,chondroitin sulfate,MSM which can be taken OTC.These substances help in repairing of damaged cartilage of knee in early stages.
-Avoid squatting movements at knee.
-In early stages exercises, medicines ,joint supplements & intra articular injections are helpful & may revert the changes but in late cases joint replacement is only option.
