Is It Advisable To Start Thyroxine Sodium 50 Mcg During Pregnancy?

Question: Hi my wife is 12 week pregnant and got TSH 3.36 at 6 week she was 3.82 TSH and 5 week at 4.82 doctor stated thyroxine sodium 50 mcg today is this ok

Yes it is prudent to start thyroxine medication. The TSH targets during pregnancy are:

0.1 to 2.5 in the first 3 months

0.2 to 3 in the second 3 months

0.3 to 3 in the last 3 months

It would also be a good idea to check thyroid auto antibodies with the next set of laboratory tests in addition to thyroid hormone levels such as:

anti TPO (Thyroid peroxidase antibodies)

anti Thyroglobulin antibodies

Total T3

Total T4

TSH

When I see someone like you in my practice , I also order the following tests n addition to a thorough physical examination:

CBC
HbA1c
Liver kidney function tests
Kidney function tests
Electrolytes

None of these tests require fasting and can be done at any time of the day or night.

You will be best served by seeing an endocrinologist in person because it will require experience and training to adjust your thyroxine doses in order to get the dose right by checking at regular intervals of time

At 6 week T4 was 19.8 and T3 was 5.1 but doctor started thyroxine at 12 weeks why not at 6 week , yes she referred to us to endcrologist
