My nephew has been peeing blood for over a year. He's 18 and has a hemoglobin level of six.(I think that's the term) he's getting iron infusions periodically and since this last test, they're admitting him for a week of infusions. Is it normal not to improve in all this time? We're worried we aren't aggressive enough in questioning doctors and seeking answers. What should we do?

The most possible underlying cause here is clotting factor disorder, platelet deficiency or other hemotological issue; which must be evaluated thoroughly for proper management.
