Dear Sir

CT is suggesting of having a tumor (exact nature can't be commented upon) in the pancreas. Biliary and pancreatic ducts are the ducts which take bile from liver and pancreatic juice from pancreas to small intestine, respectively. These ducts are not dilated. Right hepatic artery is artery which takes blood to right side of liver and SMA is artery which supplies to whole of small intestine and part of large intestine. 360 degree encasement and 180 degree contact with artery essentially means that tumor becomes un-resectable without damaging/ electively cutting. Damaging these vessels is life threatening. There are some lymph nodes which has increased in size, which may be related to tumor. Essentially, based on available CT scans, the tumor is locally advanced.

Since only CT report is available for comment, i will confine my answer to explain it only. Would suggest you meet surgeon (preferably Gastro Surgeon) with CT scan films and CD of CT.
