What Does This Blood Report Indicate?

Hi, I was diagnosed with Systemic Lupus and Psoriatic Arthritis 3 and a half years ago. I ve suffered from Psoriasis for the past 18 years. Six weeks ago, my Rheumatologist increased my Methotrexate dosage to 25 mg (2.5mg tablets x 10) from 20mg (2.5mg tablets x 8) once weekly and changed my NSAID from Naproxen 500 mg to Voltaren 75mg twice daily. My AST value on that date was 20 and my ALT value was 13. During my 6 week followup exam and labs, my AST came back as 61 and my ALT is 39. Several other labs came back abnormal. My RBC was 3.94, Hct was 36.8, MCH was 32.9 and my NE% was 68.8. I was told to discontinue the Methotrexate and Voltaren until I come back in six weeks for a repeat exam and lab. I m really concerned seeing that I m only 37, stand at 6 tall, weigh 150 lbs, pretty active and do not drink at all. I take multi-vitamins as well as additional folic acid, B6, B12, Vit. E and Biotin as ordered. I m not sure what I should do or what is causing this. Please advise.

Hello, Your liver enzymes indicate a mild transient elevation . This occurs in some patients sometimes and cannot be predicted. You have been rightly advised to discontinue your former medication . If the elevation of liver enzymes is a transient response they usually come back to normal in 6 weeks time. It is important to monitor your blood counts and liver enzymes periodically when you are drugs such as methotrexate. Right now you may continue your vitamins as advised and wait for your next lab tests. Should you require further assistance in understanding your lab reports you may post them here with your queries.
