What Does Constant Pressure In The Rectal Area Indicate?

My friend has constant pressure in rectal area and a feeling like “worms” there. That is the way she describes it! She has had a colonoscopy, MRI, and has been to the gyn and had a sonagram ( with vaginal sonagram). This condition is 24/7. Started about 4 months ago. What could it be? She had shingles about 6 years ago. Could it be a nerve problem?
