What Do White Patches In The Throat And Tongue Indicate?

59 year old, healthy and fit, female has white patches on throat, tongue appears more white than usual, and more fatigued than usual. Taking antibiotics for over a year due to an eye problem - oral minocycline 100 mg, and in eye erythromycin 0.5%. Throat is sore and more sore when swallow or eat. No other symptoms that suggest strep.

1. As there is a history of taking a course of antibiotics for a longer period [1 year], thus it could be 'oral thrush' because of that, but simultaneously other causes have to excluded like:

i. take an opinion of a dentist to exclude any dental anomaly [ill fitting denture]
ii. consult an ENT Physician [quick visual examination and strep. test] for any other causes[infective/pathological].
iii. poor oral hygiene
iv. dry mouth, addiction [smoking]
v. At her age, any underlying medical condition like diabetes, iron deficiency anemia, vitamin B12 deficiency, underactive thyroid.
vi. check with VDRL and HIV test to exclude sexually transmitted disease.

PS. rule of thumb is that if you see a white patch anywhere in the throat suspect 'diphtheria' and refer immediately.

2. In the meantime till you get an appointment, make sure:
- maintain proper hydration
- maintain proper oral hygiene [saline water gargles, and mouthwash after every meal]
- stop any addiction since they propagate the formation of white spots in the mouth.

PS. even after you have tried all the measures but the spot won't go away or recur, it is best to visit your doctor.

Thanks for the query..

White patches on throat and tongue with a medical history of taking antibiotics for an year is pointing towards oral yeast infection or Oral Thrush..

Antibiotic can kill even good bacteria that can resist overgrowth of other harmful organisms like yeast leading to yeast overgrowth causing Oral Thrush..

So first of all consult your Physician and get evaluated, if yeast infection is confirmed as the diagnosed then you can be advised anti-fungal medication like Nystatin mouthwash, Clotrimazole lozenges and oral antifungal medication like Fluconazole and Itraconazole..

Probiotics like Yogurt and garlic can also help in clearing the fungus soon..

Also the antibiotic intake should be either reduced or stopped after discussing with your doctor to reduce the infection faster..
