What Do Severe Headaches Indicate?

While studying, your normally energetic roommate seems to be more subdued than normal. Complaining of a headache, she decides to go to bed. A few hours later, your roommate gets up in obvious pain. Looking for ibuprofen, she says she must have hurt her neck while working out earlier in the day because it is really stiff and causing her headache. She also complains about your bright lights while studying and how hot you have the room. After she returns to bed, her symptoms continue to worsen over the next few hours and it becomes apparent she has a high fever. After some quick research you decide it’s time to go to the emergency room to get checked out. Your roommate is reluctant because she is currently without adequate insurance coverage and immediately begins to cry inconsolably (very out of character).

Thanks for contacting with your health concern and after reviewing your case history, I'm of the following opinion:

1. as symptoms are sudden in origin with neck stiffness, fever, photophobia thus an immediate visit to ER [emergency room] is warranted to exclude Meningitis.

PS. photophobia can also be caused by a migraine or due to an infection or inflammation that can irritate the eyes.

2. As because of studying and physical exertion did earlier it could be simple Tension headache but as a routine, I do check the following signs in my patient:

i. check blood pressure.
ii. check sinus tenderness over maxillary & frontal sinuses and look for any focus of infection in teeth, tonsils, and ears.
iii. check tenderness on the nape of neck or pain on neck movements.
iv. look for any painful/red-eye and [if present] ask for ophthalmologist opinion.

PS. An immediate Neurologist opinion and C.T. scan [if needed] is required since best treatment is to address the underlying cause.
