What Could Cause Testicular Pain?

my left testical aches, has been hurting on and off over the last couple of months and for 1 week my semen looked like it had cottage cheese in it. It has since cleared up but the pain comes back off and on. The pain sometimes leads up to my hernia and this might sound weird but during the times it is hurting I can press on my left nipple and it will can activate the pain down to my left testicle.
