The Dominican republic is a place where you are always in danger to get infected.
Schistosomiasis is very common. You can take it by swimming or being in contact with natural waters where snails are seen(snails act as an intermediate host).
It may affect the
spinal cord and cause
back pain but also it can damage the
liver as well. It very important to treat it accordingly because the infection may turn to chronic and the consequences are very dangerous.
When it goes as a chronic disease it develops months or years after the first infection. You may be unsymptomatic till the infection or better say the eggs of the parasite travel throughout the body-heart, liver, lungs, brain.
Also,
hepatitis B is very common and maybe you have got it there. Sometimes when associated with other virus infections hepatitis may progress faster leading to liver enlargement and liver insufficiency in a short period of time.
Bladder infection is usually caused by
E.coli but it has many other causes. Theoretically, all parasites and bacteria may cause it. A urine culture will distinguish the exact cause.
A spine CT and maybe
MRI will be needed to distinguish the levels of damage in the spine and the causative agent. Maybe it is not just a muscle spasm.
My advice is to be first in contact with an infectious disease treating medical center for further evaluation.
