Hi..
Welcome to HEALTHCARE MAGIC..

I have gone through your query and can understand your concerns.

As per your complain the white patches in mouth seems to be due to leukoplakia as you have a history of tobacco chewing and smoking..
Red patches can be due to inflammation inside mouth causing stomatitis and as the patches have started lightening up it does not seems like Cancer..
You should first of all completely stop smoking and chewing tobacco and certain aids like Nicotine chewing gums and patches can help in reducing the urge to chew and smoke..
Continue taking the medications that are multivitamin and antioxidant supplements given to you by the doctor and for red patches if they are painful gargle with numbing mouthwash, suck ice and do salt water rinses..
Avoid spicy food..
Maintain a good oral hygiene..
But in case if you continue chewing and smoking tobacco there can be oral cancer..

Hope this informations helps..

Thanks and regards.
