What Causes Intermittent Fever, Vomiting And A Fall In The Platelet Count?

Hi. My wife age 29 is detected positive of ns1 antigen. She started with fever 3 days back . In 3 days her platelet count reduced as 110 then 94 theb today was 82. She is taking lots of fluid and paracetamol for fever sos. Please suggest further trends of dengue. No rashes No bleeding. She vomits occssionally in fever above 101 fahreheit. Please advise future trend and alarming stage. Also advise can platelet count drop more than 20000 in aday??

After going through your question, I can understand your concern.

There is dengue fever in your wife's case.

Complications associated with dengue are-

1) Low platelet count (Thrombocytopenia) this is due to platelet destruction and bone marrow suppression (decreased synthesis).
2) Coagulopathy: bleeding tendency.
3) Hepatitis and acalculus cholecytitis (inflammation of liver and gall bladder)
4) Kidney failure.
5) Shock low blood pressure.
6) Respiratory distress.

So there may be a need for following depending upon clinical evaluation -

- Daily platelet count.
- Complete Blood Count.
- Liver function Test.
- Kidney function test.
- Chest xray.
- Ultrasound of abdomen and pelvis.
- Coagulation profile.
- Urine examination.

Keep watch on-
- Bleeding from any site urine, nose, while brushing, patechie and rashes.
- Breathing difficulties.
- Abdominal pain, vomiting, jaundice.
- Dizziness, change in level of consciousness.

Depending upon her clinical evaluation and above test results there may be need for hospitalization.

Use paracetamol for fever and maintain adequate hydration.
