What Causes High Blood Pressure And Heart Rate When Diagnosed With Diastolic Dysfunction?

I am a 35 year old female. 4 years a go I was diagnosed with diastolic dysfunction of the left side. I completed a stress test and the cardiologist stated it appeared fine. I did not follow up in a year. Most recently i have been having dizzy spell/feeling light headed. It used to only last a few seconds and accompanied by a very breif period of confusion while I refocused, never lasting more than 30 seconds. Today I woke up and felt bad ( originally blamed it on the extra glass of wine from the night before) but it got worse and I felt off balance. My BP was 140\107 so I went to local urgent care. My bp remained high even after I took lisinopril 20mg (not something I usually take). They did and ekg and it appears that i have left atrial enlargement. My resting heart rate has been on the 100 for the last few months and was 95 during the ekg. Is the left atrial enlargement due to diastolic dysfunction? Or high BP. I feel like my BP is very high but doctors are not concerned by it. I have made an appointment to see a cardiologist in a few weeks but wanted to see if anyone could explain this more to me. I have a medical background but this is outside my scope of practice.

Hi,

Based on the history you have shared, it can be cardiac syncope that you experienced recently.

It's better to get holter monitoring to rule out any transient arrythmia or abnormal heart rythm during the course of the day so 24 hours monitoring of EKG through holter is a must.

Also electrolytes and hormonal or metabolic profile like thryoid and sugar levels may be needed.

BP is higher than normal and that may have lead to left side heart enlargements however echocardiogram may be needed to rule of valvular lesion or stenosis.

You can use diuretics like hydrochlorothiazide alongwith losartan meanwhile.
