What Causes Extreme Migraines?

Hello, I am a 32y/o male, 11 years ago, I fell from 25ft. I suffer from extreme migraines, numbness in fingers if I straighten my head (tilts to the right), when looking to the right I have to turn my whole body, looking to the left causes blurred vision (MRI shows C5 Bone Spur). For my back, I get insane muscle spasms that leave me in the fidel position for hours, tingling in toes along with pins & needles down my legs (MRI shows L4/L5 disc bulge & L5/S1 disc herniation with inferior extrustion). On top of all that, I have RSD in my left foot. Been to Physical Therapy & was sent to the ER 2x in 1 wk. Needless to say, I do not go anymore. I have been to the pain clinic (not helping) & tried of taking pills. Have already seen the Neurosurgeon which wants me to see the pain clinic 1st. Do you have any suggestions?
