What Causes Ear Wax Flowing Out?

Hi Doctor,

My 4 year old grandson's ear wax has been running out of his ears for two days on and off, the left being worse, small drainage on pillow last night. Last year had tonsils out and tubes put in both ears. Has happened a couple of times before. Should I be concerned about this?
