What Causes Ear Pain Following Sinus Infection?

I had a sinus infection and ear pain that was treated with amoxicillan. The sinus infection went away. The ear pain did not. And the extreme fatigue I had been feeling during the sinus infection continued. I was put on a topical steroid for the ear. No improvement. I was referred to an ENT and during that appointment it was noted that my blood pressure was extremely high - 163/118 - I have normally very low blood pressure, no family history of it, no key risk factors. After EKG and lab tests and tracking BP for several days,I was put on a diuretic to control the blood pressure. Meanwhile, back at the ENT appointment, he could see nothing wrong with my ear, and decided it was referred pain from my TMJ (which has never given me any trouble before). My blood pressure is now down after about 1 week on the medicine, my ear is still extremely painful, my Eustachian tube feels full and and my tongue feels cumbersome. I have lost about 6 lbs and continue to be extremely fatigued - which I assume is due to the medication (although I was quite fatigued the month before from the sinus infection and ear issue). It s hard to believe the ear pain is from TMJ, and that these two things (BP and ear issue) are unrelated. I appreciate any thoughts.

Hi,

The commonest causes in such a case of your symptoms are:
* Trigeminal neuralgia instigated by a vascular loop around the nerve in its course and started with due to hypertension causing pain. Although the blood pressure gets normal the pain can continue on which I hope will settle with time. Eustachian tube dysfunction can cause such pain on intermittent basis.
* Continued sinusitis
* TM joint swelling

Any other cause internally that may be missed on routine examination even on endoscopy. Hence I would advise you tto consult your ENT surgeon again and ask for MRI for trigeminal nerve and vascular loop around. Get CT scan for sinuses and get definite treatment, medical or surgical and get rid of it. Continue treatment for blood pressure, monitor blood pressure and its relation to intensity of pain.
