What Causes Back Pain And Difficulty In Passing Urine?

hi-just had a urine test for a UTI as pain(but not the usual burning)when trying to pass water,not passing much even though I am drinking adaquate amounts-pain in right side front and back near kidney, foggy headache,body aches and pain in legs especially, general fatigue and feeling totally drained, light headedness, feeling and sweating profusely but body temp. reads 38.5 C.My urine test results are:ketones 40mg/dL, proteins-30mg/dL, sg-1.010, ph-7.5, colour-yellow, leu-trace. Is this an infection and why does it keep re-occurring as I follow strict personal hygiene, front to back and all that....was prescribed Trimethoprim again-had this last time but did not do much good as this has re-ocurred within a month and a half.
