Look, From the report, you can also notice some abnormalities like presence of protein, blood, leucocyte, RBCs,Epithelial cells.
But just from this, no comment should be done. Clinical correlation is the most important factor for the diagnosis.
As far as the report is concerned, the condition you are suffering from is most probably, nephritic syndrome, but, for its exact categorization, other parameters like lipid profile and total protein should be considered.
