Ultrasound Shows Blighted Ovum. Rising Beta HCG Levels. What Is Going On?

Not sure if I have a viable pregnancy. 1In the week of JUly Mon 15 I got light spotting for my period. but that period was due the week before. then Mon Aug 5,6,7,8,9 I should have had my perion. Nothing. tues sept 3. blood test is positive for pregnancy. hcg is 553. sunday Sept 1, I had extreme pain in my abdomen . Hard to walk. Very uncomfortable. Sept 13 hcg 4214 Im not sure how far along the pregnancy i am . but i tell the ultrasound tech my lmp was July 15. Ultrasound sept 13 says. Cystic structure with echogenic rim, MSD measures 7.7 mm , no YS and no embryo seen Mild Lt adnexal tenderness. No free fluid seen. Finding may represent early pregnancy, ectopic pregnancy can not be rule out. Ultrasound Sep 18. Possibility of single intrauterine gestational sad of 6.3 weeks size. No yolk sac or embryo visualized yet. Adnexae unremarkable. Failed early intrauterine pregnancy. Ultrasound done Sept 23..HCG 8020. its rising. PREGNANCY AND TRANSVAGINAL ULTRASOUND:Ultrasound showed an intrauterine gestational sac about 6 weeks size. It is slightly flattened. Decidual reaction is poor. There is no embryo or yolk sac seen. By dates the patient should be 10 weeks. No adnexal masses were seen. IMPRESSION: this is likely a non-viable gestation (blighted Ovum). However, if a beta hcg levels are rising appropiately then a repeat Scan canbe considered done in 2 weeks. However if the beta hcg level is falling off then this would confirm the diagnosis of a blighted ovum. I dont know what is going on. I think my pregancy is not as far along as my lmp indicates, cause i cant remember. But the blood work should be able to advise along with the size of measures gest sac. Pleas help

OBGYN 's  Response
Hello, I would be happy to help you with your question.

With a positive pregnancy test 4 weeks ago, you had to be about 3-4 weeks along. By 6-7 weeks, ultrasound should easily detect a fetus with a heartbeat. So this is clearly NOT a normal pregnancy. Even worse, this is quite possibly an ectopic pregnancy because of the adnexal pain and rising HCG levels. The best way to sort this out is to do a D&C. If there is no pregnancy tissue on the D&C specimen, then this IS an ectopic (which as you know is a life threatening problem). So you need to see an OB/GYN NOW!!!!

I hope that this helps!

Radiologist Dr. Vivek Chail's  Response
Hi,
Thanks for writing in to us.

There appears too be a slight confusion with your dates.

The final observation is that there might be a blighted ovum (non viable pregnancy) if the HCG level shows a decline in your next scan. On the other hand if the HCG level is rising and there are observable changes in the gestational sac as visualised on your scan, possibilities of a normal pregnancy may be considered.

Note: If there is very high HCG which is not coming down and also not at par with your gestational sac in the presence of other features, a molar pregnancy must be excluded. This is very rare occurrence and may not be happening in you.

Hope this helps
