From your history, it seems that you have got some inflammation of your testes which most commonly occurs due to any unnoticed trivial trauma occurring during any kind of activity if the testes are not properly supported. If I were your treating doctor, I would advise you for a urine routine/microscopic test to rule out a urinary tract infection followed by a culture sensitivity test for appropriate management. It is also logical to have an ultrasound of kidney, ureter, bladder and testis done to rule out ureteric calculi. I would also advise you for a proper scrotal support, along with an anti inflammatory drug with a proton pump inhibitor, and a broad spectrum antibiotic which may change depending on the urine culture/sensitivity test.
