Yes, a person can care for
high cholesterol on their own at home and the more social/family support, the better. Her situation is fairly complex and can be best addressed by a visit with a dietitian.
The entire lab profile, not just her cholesterol, should be reviewed as the type of nutritional recommendations made are tailored to the type of profile the patient exhibits.
Lack of sleep over time become quite serious, but nothing the primary physician cannot handle. Please monitor her temperatures. If they reach 103, take her to the hospital, otherwise, document her symptoms and contact the doctor.

Please relax. It is not the cholesterol but rather the condition of her heart that is of greatest issue. The cholesterol only contributed to the problem.
She needs something to help relax her and relieve the body aches. Some of the HCM doctors recommend lemon balm tea, but without an assessment and knowing more about your sister-in-law it is difficult to make any recommendations.
