Suggest Treatment For Severe Anxiety And Painful Tremors In The Hands

I m a 30 year old female having several diff symptoms at diff times, however I also suffer from extreme anxiety that is not currently being treated due to the fact I m in treatment at the moment at a methadone clinic currently dosing 30 milligrams daily, and currently tapering. One of my symptoms is a dull ache in my left side, not too low but not too high either, this has been going on for a couple of years but never gets painful enough to warrant going to the doctor. Also my more recent symptom is tremors in my hands, not bad just barely starting to really notice it but it even causes me to some times type the wrong letter and have to go back so its a little noticeable. I m always scared I m sick or dying hence the untreated anxiety. so I m scared, be gentle lol

General & Family Physician 's  Response
Hello,

As per the information provided by you, the symptoms including the tremors seem to be related to your anxiety.

Although another differential diagnosis for tremors can have the possibility for that detailed history is necessary.

I would suggest you some lifestyle adaptations to manage your anxiety before we head on to the pharmacological treatment.

1.)      Regular physical exercise- It will set a routine for you and your body will release endorphins that are the best stress buster hormones not less than medication.

2.)     Relaxation techniques: Certain techniques can help reduce symptoms of anxiety, including deep-breathing exercises, meditation, yoga, and full night’s sleep.

Don’t think that you are having something major or you are heading towards dying. Anxiety is just exaggerated bodily response of a normal feeling. By proper conscious learning, you can develop proper sub-conscious response system.

3.)     Support network: Try talking to a person who is supportive, such as a family member or friend. Please avoid storing up and suppressing anxious feelings as this can worsen anxiety that you are experiencing.

4.)     Avoid taking alcohol, smoking and caffeinated beverages. Counselling and therapy. Another way of managing anxiety is psychotherapy. This includes CBT i.e. Cognitive behavioral therapy.

By the counselling sessions, things are worked out to recognize and alter the harmful thought patterns that can trigger an anxiety episode and uneasy feelings, limit over thinking, and change the scale and intensity of reactions to stressors.

You can plan your psychological sessions with psychotherapy professional.

Medications: For initial therapy, you may try Imipramine with an initial dose of 75 mg and titrating further with physician advice as per response.

Plus Propranolol that will also help with tremors with a dose of 40 mg twice daily. But don’t take it if you have a history of low BP or asthma.

I recommend you to try the behavioural methods first before starting any pharmacological treatment as detailed history and counselling is necessary before starting and treatment.

Hope I have answered your query. Let me know if I can assist you further.

Take care

Regards,
