Suggest Treatment For Irregular Periods Caused Due To PCOD

Dear Doctor, I had irregular periods since I started my menstruation at the age of 15, when I was 17 i went to gynic and she gave me some medicines and said its a normal problem and will get over after marriage, when i was 26 i again consult the best gynic of our town and she dignosed POCs in my overies, so she kept me under medication for 3 month, i got regular periods for 3 months and after that for 1-2 months mre, after that again istartyed facing irregular periods cycle, so i never used to remember my periods date, i have to be prepared, sometimes i used to get long periods which lasted for 15 days or more, i got married whrn i was 28, we used precautions in the first year of our marriage, no pills, pills were taken only 2 times in that yera, the second year of marriage we went to gynic, she also diagnosed POCS in my overies and gave MEPRATe for periods, and suggested not to delay for pregnency, so we accepted her advice and tried for 2 months, i didnt get periods so i thought that m pregnent, but when we consulted the gynic and got the pregnency test done , we found it negative, now m worried, i have only one problem of irregular periods related to POCS, rest i dont have any other symptoms like unwanted hair grownth or overweight, etc, please suggest mw what to do to get pregnent and deliver a healthy baby

I understand your concern
PCOS is very complex disease and cause hormonal imbalance (androgen excess), insulin resistance/glucose intolerance and problem in conceive.

Consult the gynecologist and undergo USG scan, ovarian follicle study and reproductive hormone analysis, blood glucose and insulin, lipid profile for better management.
Main aim of the treatment is to regularize the period, maintain glucose homeostasis and maintain fertility

You have to take treatment for at least 6-8 months but period will varies depending upon the response.

Progesterone pill: in last 7-8 days of the period to induce period and regularize the period
Clomiphene citrate to induce ovulation and maintain fertility: in the first five day of the cycle
Spironolactone and flutamide: diuretic action and also induce ovulation
Metformin: for induction of ovulation and maintenance of glucose homeostasis
If medicine will not responds in next 7-8 months then IVF is ption for the pregnancy

Take healthy diet, green leafy vegetables, fresh fruits, drink plenty of water
Avoid fatty/spicy/salty foods
Avoid excessive sweets
Start regular exercise for 45-60 minutes in the morning
