Hi I'm 53 years old, female, I suffer from suspect allergic aspergillosis and congenital bronchiectases. I often spit sort of stones, their cytological examination says there are many polimorphonucleates, is there any relation between the latter and the formation of lithi?? Thank you bery much patrizia from Italy

Hi Dear !! Thanks for your query to HCM .
Read and reviewed your query and health concerns.
You seems to suffer from-ABPA(Allergic Brohcho-pulmonary aspergillosis) with congenital bronchectasis..
Its rather the existence of Congenitial Bronchiectasis-with cavitation, which has predisposed you to ABPA with sputum stones/ or aspergillosis neutrophilic granulomas.
Treatment- would be mainly with Itriconazole with Steroids,to reduce the impact of allergic aspergillosis in broncopulomonary segments.
These lithis in the sputum, are actually of thick mucoid balls with neutorphils and macrophages in the microscopic studies.
This is because, neutrophils(polymorphoneucleocytes) and macrophages attack the aspergillosis fungal infection, which occures with reduced immunity from bronchiectatic infected cavities.
So the reply to your query-That these lithis are directly related to the polymorphoneucleocytes found in the sputum.

Plenty of fluids and Postural drainage with bronchial drainage to reduce the bronchectatic infection of the aspergilous fungus,is the main basis in whole of the treatment.

Asthmatic Symptoms treated by general anti-asthamatic protocols-like bronchodilators/ antibiotics ect.
Non-resolving conditions -may require surgical excision of the affected segment of the lungs.
Regualr check X-rays showing knew shadows with CT studies would fix the APBA- illness in cases like you
A Team of Chest and allergen Specialist with Chest Surgeon and Physician is needed to investigate various accompanied factors-like diabetes, existing TB lung infection with bronchiectatic cavitis, HIV infections and to treat these accompanied factors to resolve the asthmatic complication in cases like you.

Hope this would help you to plan further care of this complex illness of yours.
