Suggest A Solution For Severe Leg Pain For An 87-year-old Woman

My 87 year old Aunt was suffering with a kidney infection about a month ago. In the process of being treated with antibiotics, she continued to feel worse. She complained of aching in her legs. It became difficult for her to walk unassisted. Before this, she has been fully ambulatory taking care of her house work and cooking for her husband and herself. About 7 days ago, she fell while going to the restroom. She was admitted to the hospital to be checked out. At that time they found she had a white blood cell count of 36,000. Since that time they have done all kinds of test and haven t found anything that would be causing such a high white blood cell count. After seven days in the hospital and lots of fluids and antibiotics, they still haven t found an answer. She appears to get weaker everyday. She has not had a bowel movement about two days before her admission to the hospital. She had a catheter inserted upon arrival. Two days ago they tried removing the catheter but she now cannot relieve herself. They have had to drain the bladder several times and now have reinserted the catheter full time. Her legs seem to get weaker by the day. They have done a complete body scan, MRI of her spine, spinal tap, sonograms, blood tests . . . Do you have any ideas?????

General Surgeon 's  Response
Hello,

The presenting situation with swelling legs, pain, inability to walk after recent hospitalisation for kidney infection may have various underlying reasons as:
- Deep vein thrombosis (from bed ridden condition)
- Systemic sepsis
- Advancement of renal failure
- Associated heart failure
- Fluid overload

However, if the last reports of discharge time if available as total white cell count, serum creatinine, Doppler lower limb and else are available, it would be of best help to understand it more detailed way.

Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Bhagyesh V. Patel, General Surgeon
