My Brother is a alcoholic. He has been drinking for about 35 years... He has very high ammonia level and is on his way out. Throwing up blood put him in the hospital and he just got out .. He will be drinking again soon .. I think if he has one more drink he will be gone. A slow suicide ...

The first step and the main one is motivation to quit. Some hope / optimism for the future might be an important missing step; being loved/valued by the family is not missing but might be further emphasized?
In end-stage liver disease, quitting might not help the limited life span all that much, but cannot assess it without quitting. Have had (50/50?) patients with all those symptoms who got better, some who didn't.
