General & Family Physician 's  Response
Hi and thanks for the query,

It would be necessary to know if this diagnosis of glandular TB was made on a presumptive basis or a confirmed histopathological basis. This precision is important for failure to respond to appropriate treatment should cause other diagnostic possibilities to be considered. A tumor like a lymphoma has to be carefully considered in this case. In case TB was confirmed, screening for systemic diseases like systemic lupus erythematosus, rhuematoid arthritis, sarcoidosis and other systemic diseases would be very necessary. A lymph node biopsy and an HIV test would be useful . General swelling calls for investigation of kidney function. Kidney function tests should be run. An abdominal ultrasound in search for abdominal lymph nodes, a chest X ray and appropriate ultrasound could be useful.

I suggest a complete reevaluation by an internist. thanks and kind regards,

Bain LE, MD.
