Scan Done. What Does Result Mean In Simple Language?

Please, what does this mean in layman's terms (simple english)?
There is a large mediastinal node adjacent to the junction of the left brachial cephalic vein with the SVC. This is difficult to distinguish from the adjacent vein in the absence of contrast. It measures in the range of 2cm in diameter. No other definite mediastinal or hilar adenopathy is present. No axillary adenopathy is demonstrated.
There are multiple small scattered ill-defined pulmonary parenchymal nodules bilaterally, with the largest, in the right lower lobe inferiorly, measuring 15 mm in diameter. There is no pleural effusion.
The right hemidiaphragm is elevated. This is present on the November 2012 chest x-ray, but not on the chest x-ray of 2010.
Impression: Multiple ill-defined pulmonary masses, suggesting metastases given the history of malignancy. Upper mediastinal adenopathy. Elevated right hemidiaphragm as noted on the chest x-ray of 2012 suggesting possible phrenic nerve paralysis.

Dear patient,

There is a small, indeterminate growth between the upper veins of the chest. There are multiple small growths in the lungs that are suspicious of cancer seeds from elsewhere. The muscle that helps with respiration is partially paralyzed, likely due to damage of the nerve that makes it work.
