Hi,
Welcome to HCM
You are suffering from CHF( Congestive Heart Failure), you have not pointed out which disease of heart caused it as you are only 34 years.
There are many many diseases which cause CHF.
Your disease must be compensated for so many years now as you walk gets tired and may be short of breaths is due to increased heart failure or fluid accumulation in body because of Failure.
as time pass it may become decompensated but see a cardiologist and take treatment to clear any edema which will remove load from heart and help to recover.
Get investigated by blood test and a X ray chest to see size of heart and any swelling in lungs.
alog with a Echocardiography to exactly know function of heart (ejection Fraction).this will also tell you is it heart muscle disease or valve of heart are damaged.
Good Luck to you
