Is An ESR Of 59 And MCHC Of 30.5 A Concern?

Is an ESR of 59 and a MCHC of 30.5 significant? All other tests are within normal range. I have been experiencing weakness, extreme fatigue, joint pain, hair loss, memory/concentration issues. I am female, 56 and diabetic (Type 2 with insulin pump)

General & Family Physician 's  Response
Hello,
Thank you for asking at HCM.
Not always an abnormal result is real and we need to worry about. Generally when we have lab results that are abnormal it is safer to repeat them in a couple of weeks to confirm the results and make sure they are real values. So I would recommend you to repeat both ERS and Complete Blood Count which is the test that checks MCHC.
The ERS normal values are 20 mm/h and in your case 59 mm/h means your ERS is higher than we expect it to be.
MCHC normal values are 33-36 g/dl and in your case it is mildly reduced also which means you might have some anemia also. Most important red blood cell indices in the Complete Blood Count are RBC , HB and HCT so it would be very helpful if we had those results too.
It would be also helpful to know what tests were normal but if you repeat your tests and they come the same (high ERS and low MCHC) while you have fatigue, joint pain, hair loss and reduced concentration you might need to have a visit with a doctor and rule out some diseases called autoimmune diseases. Your doctor might order those tests for you.
It might be helpful to also do your routine screening tests like mamograme, colonoscopy and pap smear if you have not done them routinely as recommended. If you have never checked your thyroid I would also recommend a TSH check. Regularly checking your glucose, BP, urinalysis, creatinine, lipids, ECG, eye examination, is helpful also as a routine check for patients with diabetes and hypertension.
So to finalize it:
1. repeat the tests in a couple of weeks
2. if the same values you will need a visit with your doctor which might do routine screening tests for age and gender (mamograme, colonoscopy, pap smear), testing for autoimmune disease and routine testing in patients with diabetes and hypertension (lipids, glucose, creatinine, eye exam, urinalysis, ECG/heart ultrasound, ect) and other tests that the doctor might find necessary after examining you.
I hope this is helpful and answers to your question but please let us know if any questions or concerns.

Best regards,
I find this answer helpful
